Mobile Medical Care 

HAMSMART
Hamilton Social Medicine Response Team (HAMSMART) is a mobile outreach service of general internal medicine and infectious disease physicians, registered nurses, midwifery and outreach workers whose goal is to provide medical care to individuals with barriers to accessing care in the traditional medical system.

Wesley Urban Ministries
Health care program for men and women experiencing homelessness, addiction and/or serious health problems as a result of addictions. Residential and outpatient services available (fees may vary depending on income). Services offered include:
  • harm reduction strategies
  • 24 hour nursing care
  • physician care
  • psychiatric care
  • 24 hour social services
  • counselling
  • housing
  • health assessments and monitoring
  • stage-based motivational strategies

Shelter Health network
Provides primary care to patients at several locations in Hamilton, including shelters and transitional housing programs, drop-in centers, addiction facilities, and mental health facilities. Designed to assist patients who face barriers in accessing the traditional health care system, including patients who do not have health cards, do not have a phone or address to receive appointment information, or who feel stigmatized or unsafe when they access care in traditional settings.

​Referral Process: anyone who is experiencing homelessness or precariously housed and does not have a family doctor can access the service by calling the clinic coordinator
